The goal of Heart Care Plus+ is to improve community and population health by enhancing the scope, quality and value of patient care throughout Wake County. This collaboration will give patients access to exceptional providers, outpatient practices and facilities, and hospitals that will work together to provide high quality coordinated cardiovascular care for patients across the broad spectrum of heart and vascular disease.
WakeMed and Duke Health are two like-minded organizations that share the goal of enhancing heart care services throughout Wake County in ways that neither Duke nor WakeMed could achieve alone. This collaboration establishes a new and innovative model of care that unites the strengths of each organization to improve access and deliver a broader scope of highly-coordinated care with greater value and affordability in Wake County while allowing both WakeMed and Duke Health to maintain their organizational independence and identities.
Heart Care Plus+ spans the spectrum of heart disease from prevention to complex care. This collaboration will make needed specialty heart services available to patients at the most medically appropriate and convenient locations in Wake County. If you are a current Duke or WakeMed cardiology patient, please know that there will be no immediate changes in the way you access the provider you have visited in the past. If you need an appointment, please call the same office and request the same provider you saw before. Once Heart Care Plus+ is fully operational this summer, more information will become available about how you can access its comprehensive services and offerings.
